EGR coolers commonly fail because of a clogged oil cooler that is keeping any coolant from passing through the unit. The square-shaped Powerstroke 6.0 EGR cooler is known to clog due to its small exhaust inlet and outlet. White exhaust smoke, coolant in the intake manifold, and coolant loss from the degas bottle are symptoms of a bad EGR cooler.
